Birth Certificate 2 Proofs of Residency – Example: lease agreement or utility bill (phone bills not accepted) Social Security Card Custody Papers – If a child is not living with both biological parents, or if a child lives with only one parent, proof of legal guardianship or custody papers is required and must be singed by a judge.
